Data Ltd.) has already mailed you a personalized claim form that has your loss data, as established by Morgan Keegan, pre-loaded into the form. Please review the personalized claim form sent to you by the Fund Administrator before trying to use the generic forms. If you agree with the loss data, then all you have to do is follow the instructions for signing and mailing the form back and you will not have to provide any additional documents to prove your claim (this will save you a lot of time and effort). If you do not agree with the loss data on the personalized claim form mailed to you by the Fund Administrator, then feel free to use the hyperlinks to access and print off a generic claim form.
